Transition Leadership Institute 2024

June 9-11, 2024

Connectedness connects the dots, makes the link between the seen and unseen, builds bridges, and helps others see connections and purpose in everyday life.

The Transition Leadership Institute is a jam-packed, two-day professional learning opportunity for school district teams, their partners, and parents to come together to learn evidence-based transition practices from national, state, and local experts. Sessions are designed to align with this year’s theme of “The Power of Connectedness.” The goal is to strengthen effective practices and grow and the following areas:

  • increase knowledge of effective transition practices;
  • use relevant data to reflect on current practices;
  • update or develop interagency strategic plans;
  • foster youth and parent engagement; and
  • develop new and strengthen existing partnerships among educators, community partners, and families.

Team & Registration Information

  • One important focus of the 2024 Transition Leadership Institute is the importance of connectedness and ensuring that a collaborative, interagency team works together to ensure that students and families are connected to appropriate support when students exit the K-12 system. 
  • This institute is for school district teams of 2-8 individuals. Each team member must register.
  • All teams will leave the institute with an action plan for how to support effective practices. 
  • Parents do not need to be members of a school district team in order to attend. There is a strand to support parents in the transition planning process.

Registration closes two weeks before the event. No late registrations will be accepted! Registration will close on May 27th, 2024.

Team Considerations

You may consider including the following representatives on your team:

  • General and special education teachers,
  • Division of Vocational Rehabilitation (DVR) or School to Work Alliance Program (SWAP)
  • Case Management Agency (previously known as a community-centered board)
  • Independent Living Center
  • Employer
  • Workforce Center
  • Parent

Note: You will receive CDE professional development credits that can be used to review educator licenses. You may earn full-day credit only!
