The following UIP Templates and Addenda were used in the 2011-12 planning year and are now considered "retired."

UIP Templates
UIP Template for Districts (Updated: September 6, 2011 - V. 3.3)
The district template is intended to address state accreditation, dropout designation and federal (e.g., Title IA, IIA, III, special education) improvement planning requirements.
- Added AMAOs and HQ to worksheet on page 6.
- Updated AMAO 2010-11 Expectations on page 3.
- All districts are required to use the template in 2011-12.
UIP Template for Schools (Updated: August 9, 2011 - V. 2.1)
The school template is intended to address state accountability and Title I improvement planning requirements.
- All schools are required to use the UIP template in 2011-12.
UIP Template for AECs (Updated: September 27, 2011 - V. 2.1)
The school template for Alternative Education Campuses (AECs) is intended for those schools designated as Alternative Education Campuses by the State Board of Education.
- The school template for AECs meets all of the same requirements as the template for traditional schools. It is similar to the UIP template for traditional schools, except that it accommodates for the four key performance for which AECs are accountable: achievement, growth, postsecondary readiness, and student engagement.
- All AECs are required to use the AEC UIP template in 2011-12.
- Section I of the template has been populated with minimum state expectations. AECs will, however, have to enter their own school's results on state and federal accountability in Section I, given the option of adding additional district or school-specific AEC measures.
UIP Addenda Forms
- A new feature has been added to the UIP templates – the addenda forms. For schools and districts with unique requirements (e.g., Title I programs), the forms provide an easy-to-use format to ensure that all requirements are met by pasting the form to a completed UIP template. Schools and districts may use more than one form, if appropriate. Some schools and districts may meet some of the requirements in earlier sections of the UIP. This form provides a way to ensure all components of the program are met through (1) assurances, (2) descriptions of the requirements or (3) a cross-walk of the Title I program elements in the UIP. Note that districts identified through Title IIA (2141c) are required to use the Title IIA (2141c) addenda form.

All districts identified under Title IIA (2141c), must completed this form and include it with the district UIP.

Grantees that identified for improvement (missed AMAOs) under Title III may choose to use this format to document requirements.

The Colorado Department of Education (CDE) created the Unified Improvement Plan (UIP) to streamline improvement planning efforts. The UIP incorporates state and federal accountability requirements into a single improvement plan, and provides a roadmap to assist districts and schools in this work.
Plans were first submitted to CDE for publication on April 15, 2011. CDE reviewed and approved district turnaround plans and plans identified under ESEA programs (i.e., Titles IA, IIA, and III), and provided support as requested in the development of all other plan types. The published plan may reflect revisions districts and schools have made to their plans following official submission to CDE each year on April 15. As planning is an ongoing process, these documents reflect a "point in time" submission.
