Unified Improvement Planning - Templates
Updates:
- The district and school-level improvement planning templates and addenda forms for the 2012-13 planning year are now posted below. This includes the UIP template for Alternative Education Campuses (AECs).

2012-13 UIP District-Level Template and Addenda Forms:

UIP Template for Districts (Updated: June 28, 2012 - V. 4.1)
- The district template is intended to address state accreditation, graduation district designation, grant requirements (e.g., District Partnership Grant, ISP Grant), and federal (e.g., Title IA, IIA, III) improvement planning requirements. Version 4.1 corrected a couple of minor typos -- the changes were minor enough that districts that downloaded version 4.0 should not need to update their templates.
- All districts are required to use the current template.
- Some districts are required to use the following addenda:

District Turnaround Status Addendum (Updated: June 19, 2012 - V. 2.0)
- Districts on Turnaround status are required to complete this addendum as part of their improvement plan.

Student Graduation and Completion Plan Addendum (Updated: June 19, 2012 - V. 2.0)
- Districts that are designated as a Graduation District are required to complete this addendum as part of their improvement plan.

Title IA Addendum (Updated: June 21, 2012 - V. 2.0)
- Districts accepting Title IA funds and are on Turnaround or Priority Improvement plan types are required to complete this addendum as part of their improvement plan.

Title IIA Addendum (Updated: June 19, 2012 - V. 2.0)
- Districts accepting Title IIA funds and are on Turnaround or Priority Improvement plan types are required to complete this addendum as part of their improvement plan.

Title III Addendum (Updated: June 19, 2012 - V. 2.0)
- Districts accepting Title III funds that have been identified for improvement may select to complete this addendum as part of their improvement plan. (Optional)

Template for Schools (Updated: June 28, 2012 - V. 3.2)
- The school template is intended to address state accountability and certain grant requirements (e.g., TIG, ISP, Title I SIG). Version 3.2 corrected a couple of minor typos -- the changes were minor enough that schools that downloaded version 3.1 should not need to update their templates.
- All schools, including new schools, are required to use this template.

Template for AECs (Updated: December 17, 2012 - V. 3.3)
- The school template for Alternative Education Campuses (AECs) is intended to address state accountability and certain grant requirements (e.g., TIG, ISP, or Title I SIG. The template was updated on December 17 with the state expectations for AECs inserted.
- All AECs, including new AECs, are required to use this template.

Title I Schoolwide Program Addendum (Updated: June 12, 2012 - V. 2.0)
- Schools that participate in Title I are required to use this addendum form to document Title I program requirements for operating a Schoolwide program.

Title I Targeted Assistance Program Addendum (Updated: June 12, 2012 - V. 2.0)
- Schools that participate in Title I are required to use this addendum form to document Title I program requirements for operating a Targeted Assistance program.

Gifted Education Program Addendum (Updated: November 12, 2012 - V. 1.0)
- The Gifted Education Program Addendum is optional for the 2012-2013 school year. A small number of districts and BOCES have offered to pilot using the addendum this year. The Colorado Department of Education (CDE) created the Unified Improvement Plan (UIP) to streamline improvement planning efforts. The UIP incorporates state and federal accountability requirements into a single improvement plan, and provides a roadmap to assist districts and schools in this work.
Plans were first submitted to CDE for publication on April 15, 2011. CDE reviewed and approved district turnaround plans and plans identified under ESEA programs (i.e., Titles IA, IIA, and III), and provided support as requested in the development of all other plan types. The published plan may reflect revisions districts and schools have made to their plans following official submission to CDE each year on April 15.
As planning is an ongoing process, the posted unified improvement plan documents reflect a "point in time" submission.
Beginning in 2011-12, all districts and schools have been expected to complete an improvement plan using the current CDE unified improvement plan templates.
