Unified Improvement Planning - Templates

Updates:

  • All schools and districts are required to use the UIP online system template for the 2016-17 school year. This template is accessible through CDE's Identity Management System. Visit the UIP Online System webpage for more information. Contact Lisa Medler (medler_l@cde.state.co.us) with any questions.
  • Spanish translations of 2015-16 UIP templates and addenda are now available. Please contact Kelly Stritzinger at Stritzinger_k@cde.state.co.us for any other questions or concerns regarding the Spanish templates.

2016-17 UIP District-Level Template and Addenda Forms:

UIP Template for Districts  (Online System(Updated: June 17, 2016)

  • The district template is intended to address state accreditation, graduation district designation, program (e.g., Gifted Education) requirements, and federal (e.g., Title IA, IIA, III) improvement planning requirements.
    • All districts are required to use the current template.
    • Some districts are required to use the following addenda:

District Turnaround Status Addendum  (Online System(Updated: June 17, 2016)

  • Districts on Turnaround status are required to complete this addendum as part of their improvement plan.

Student Graduation and Completion Plan Addendum  (Online System(Updated: June 17, 2016)

  • Districts that are designated as a Graduation District are required to complete this addendum as part of their improvement plan.

ESEA Addendum  (Online System(Updated: June 17, 2016)

  • Districts accredited with Turnaround or Priority Improvement plan types and accept ESEA funds (i.e., Titles I, IIA, III) are required to complete this addendum as part of their improvement plan.

Title III Addendum  (Online System(Updated: June 17, 2016)

  • Grantees identified for improvement under Title III are required to complete this addendum as part of their improvement plan.

Gifted Education Program Addendum  (Online System(Updated: June 17, 2016)

  • Lead Districts and Administrative Units that operate a Gifted Education Program are required to complete this addendum as part of their improvement plan.

  • The school template is intended to address state accountability, program (e.g., CGP), and certain grant requirements (e.g., TIG).
    • All schools, including new schools, are required to use this template.

UIP Template for Alternative Education Campuses ( (Online System(Updated: June 17, 2016)

  • The Alternative Education Campus school template is intended to address state accountability, program (e.g., CGP), and certain grant requirements (e.g., TIG).
    • All Alternative Education Campuses are required to use either this template or the UIP Template for Schools.

School Turnaround Status Addendum  (Online System(Updated: June 17, 2016)

  • Schools with a Turnaround Plan are required to complete this addendum as part of their improvement plan.

TIG Transformation Addendum  (Online System(Updated: June 17, 2016)

  • Schools that participate in the Tiered Intervention Grant and selected the Transformation Model are required to complete this addendum as part of their improvement plan.

TIG Turnaround Addendum  (Online System(Updated: June 17, 2016)

  • Schools that participate in the Tiered Intervention Grant and selected the Turnaround Model are required to complete this addendum as part of their improvement plan.

TIG Closure Addendum  (Online System(Updated: June 17, 2016)

  • Schools that participate in the Tiered Intervention Grant and selected the Closure Model are required to complete this addendum as part of their improvement plan. 

 


The Colorado Department of Education (CDE) created the Unified Improvement Plan (UIP) to streamline improvement planning efforts. The UIP incorporates state and federal accountability requirements into a single improvement plan, and provides a roadmap to assist districts and schools in this work.

Plans were first submitted to CDE for publication on April 15, 2011. CDE reviewed and approved district turnaround plans and plans identified under ESEA programs (i.e., Titles IA, IIA, and III), and provided support as requested in the development of all other plan types. The published plan may reflect revisions districts and schools have made to their plans following official submission to CDE each year on April 15.

As planning is an ongoing process, the posted unified improvement plan documents reflect a "point in time" submission.
