PUBLICATION TRANSMITTAL FORMS

Publication Transmittal Form for Print Documents This form should be used when sending documents to the State Publications Library. The form can be filled in on-line and then printed for submission with the documents being sent.  At this time the form cannot be submitted electronically.

Publication Transmittal Form for Electronic Documents  This form should be used when informing the State Publications Library of electronic documents available through the Internet.  The URL of the new document will be submitted electronically.
