Important Due Date
School Accreditation and Request to Reconsider Form Due Oct. 17
Attn: Superintendents, District Accountability Contacts, Federal Programs Contacts
The School Accreditation and Request to Reconsider Form is due to CDE by Oct. 17. The form is the method for districts to indicate if you agree/disagree with CDE’s preliminary plan types for schools, based on the draft School Performance Framework (SPF) reports sent by CDE in August 2011. This is also the tool that districts use to submit schools’ accreditation ratings, if they differ from CDE’s plan types. If your district disagrees with a CDE plan type, you will need to indicate your recommended plan type and submit additional evidence for CDE’s consideration.
Districts must submit the School Accreditation and Request to Reconsider Form online and also submit a printed version signed by the district superintendent and local board chair. The printed version should be scanned and e-mailed to Liz Lenz, lenz_e@cde.state.co.us, by Oct. 17. To access and complete the School Accreditation and Request to Reconsider Form, go to: http://www.cde.state.co.us/scripts/AccreditationForm/. Your superintendent and district accountability contact received a unique username and password via e-mail from Somoh Supharukchinda on Sept. 14. Contact Somoh if you have questions about the form. For more information, including policy guidance and example Request to Reconsider proposals, visit the link below.

Update
Download Pre-Populated UIP Reports
Attn: Superintendents, District Accountability Contacts, Federal Programs Contacts
For the 2011-12 school year, all schools and districts are expected to complete an improvement plan using CDE's Unified Improvement Plan template. The school and district templates incorporate state and federal accountability requirements into a single improvement plan. To support this process, the state has pre-populated data in Section I of the UIP template for each school (pp. 1-2) and district (pp.1-4). The reports are considered preliminary since the plan types for state accountability will not be finalized until November – though they will not change unless your district is approved for a Request to Reconsider that results in a different plan type. The final pre-populated reports will be re-released in December with the final plan assignments. All other data (including AYP and Titles I, IIA, and III improvement status) in the preliminary report, however, should be considered final.
There are two options on getting your district and school reports: (1) Word version or (2) PDF version. Your superintendent, district accountability contact and federal programs contact should have received an e-mail from Liz Lenz on Sept. 30 with specific instructions for accessing the reports. For Option 1, the Sept. 30 e-mail included a unique URL to your district and school pre-populated UIP reports. For Option 2, you’ll need to download your district and school pre-populated UIP reports via CEDAR, https://cedar2.cde.state.co.us/, from the same folder as the DPF and SPF reports. For questions on how to access your pre-populated reports, please contact Liz Lenz.
