UIP and Addenda Forms

Addenda forms to supplement school and district Unified Improvement Plans (UIPs) are available on the CDE website.
After receiving such positive feedback on the forms for Title I schoolwide and targeted assistance programs, CDE has developed similar forms for other programs/accountability identifications (e.g., Title I programs, schools and districts with a Turnaround play type).
For districts identified under Title IIA (2141c), the form is required. Whereas other forms are optional. For schools and districts with unique requirements the forms provide an easy-to-use format to ensure that all requirements are met by pasting the form to a completed UIP template. Schools and districts may use more than one form, if appropriate. Some schools and districts may meet some of the requirements in earlier sections of the UIP. This form provides a way to ensure all components of the program are met through (1) assurances, (2) descriptions of the requirements or (3) a cross-walk of the Title I program elements in the UIP.

2009 Statewide Longitudinal Data Systems Grant Newsletter / Website

In an effort to keep all stakeholders informed on the progress and status of the 2009 Statewide Longitudinal Data System (SLDS) Grant, the SLDS Program team has created a quarterly SLDS newsletter. The newsletter will update the districts, CDE and other state agencies on all SLDS objectives (Capture, Link and Provide) and grant outcomes and is posted on the website at http://www.cde.state.co.us/SLDS/. The newsletters are posted at the link below.

The Colorado Special Education Advisory Committee is looking for new members. Please help spread the word that the committee is looking for dedicated people from across Colorado to advise the Colorado Department of Education assistant commissioner and state director of the Exceptional Student Services Unit on the issues of special education.
The Colorado Special Education Advisory Committee is a state-level committee mandated by federal and state law. Members are interested in the quality of education received by children/youth with disabilities. The committee includes parents of and individuals with disabilities, educational service providers, administrators and representatives from a variety of related agencies. Participation is statewide and representative of diverse disabilities.

2011-12 Colorado School Library Survey Now Open

The data gathered in the annual school library survey provides library professionals with important information for planning, evaluating and budgeting. Important Due Date
School Accreditation/Request to Reconsider Form Due Monday, Oct. 17
Attn: Superintendents, District Accountability Contacts
All districts must submit the School Accreditation and Request to Reconsider Form by Monday, Oct. 17. The form is the method for districts to indicate if you agree/disagree with CDE's preliminary plan types for schools, based on the draft School Performance Framework (SPF) reports sent by CDE in August 2011. This is also the tool that districts use to submit schools' accreditation ratings, if they differ from CDE's plan types. If your district disagrees with a CDE plan type, you will need to indicate your recommended plan type and submit additional evidence for CDE's consideration.
All districts must submit the School Accreditation and Request to Reconsider Form online and also submit a printed version signed by the superintendent and local board chair. You will need to click "Submit Final Form," then you will be directed to a printable version and additional instructions on any Requests to Reconsider. You may need to print the form at 80 percent to fit it on the page. The printed version should be scanned and e-mailed to Liz Lenz, lenz_e@cde.state.co.us, by Oct. 17.

Family Economic Data Survey Form Approval End Date Approaching
Attn: Superintendents, Business Managers, October Count Staff, At-risk Coordinators
Schools that are not participating in the Federal National School Lunch or School Breakfast program should have approved Family Economic Data Survey forms within 30 calendar days after the October count date for students who are eligible for free lunch.

Colorado Basic Literacy Act (CBLA) Update

In 2007, the Colorado State Board of Education approved three assessment instruments from which districts may choose for annual reporting of the Colorado Basic Literacy Act (CBLA) for grades K-3. Additionally, in December 2010, the state board recommended that the rules for administering the CBLA be revised to ensure alignment with the expectations outlined in the Colorado Academic Standards.
Therefore, in April 2012, the state board will simultaneously consider adopting updated CBLA rules and approve new assessment tools if necessary. The following timeline outlines next steps: -January 2012 - CDE will present a first draft of updated CBLA rules to the state board. -January 2012/February 2012 - CDE will publish revised draft CBLA rules and offer opportunities for public feedback. -March 2012 - SBE will conduct CBLA Rulemaking Hearing to establish the new rules regarding the administration of literacy assessments. Public comment will be invited. In addition, CDE staff will identify recommended literacy assessments for board member discussion. -April 2012 - The state board will adopt revised CBLA rules and approve CBLA assessment tools.
