BEST Conference Registration Form

Event:	BEST Conference (Broadband, E-rate, Sustainability, & Technology)
Date:	Thursday, August 25 (1:00 PM start) & Friday, August 26 (8:00AM - 1:00PM)
Location:	Great Divide Lodge in Breckenridge, Colorado www.breckresorts.com/greatdividelodge/
Fee:	Travel, lodging, and meals will be provided for one representative per library. Libraries with Legal Service Areas greater than 100,000 may send a second representative at no cost. Additional staff may attend at a cost of $165 for lodging and meals (to be paid by August 25th).

Registration Information

• Lodging is provided for Thursday, August 25th. If traveling more than 200 miles lodging will be provided for Wednesday, August 24th.
• For libraries with Legal Service Areas of 100, 000 or more a second staff member may attend at no cost.
• Additional staff may attend at a cost of $165 for lodging and meals (payable by August 25th).
• Attendees may opt to stay for up to 3 nights before or after the conference at the conference rate of $93 per night, plus tax. (note: attendees wishing to staying after August 25th will be relocated to the The Village at Breckenridge.
• The following meals will be provided: Afternoon Snack and Dinner on August 25th; Breakfast and Lunch on August 26th.
• Roundtrip mileage will be reimbursed. Airfare will only be reimbursed if traveling more than 200 miles.
• Credit card will be required for check-in at the hotel.
• Hotel parking, wireless internet, local and 800# calls are included with room registration.
• Rooms are equipped with a mini-refrigerator, coffee maker, hair dryer, cable TV, in-room movies (at your expense) and fluffy bathrobes upon request.
• We will do our best to meet attendee's dietary restrictions.
