Secondary Transition is the process of preparing students for adult life after they leave high school and can be thought of as a bridge between school programs and the opportunities of adult life, including higher education or training, employment, independent living and community participation. Transition planning provides opportunities for students with disabilities to experience positive post-school outcomes, such as:
- higher graduation rate
- lower dropout rates
- increased enrollment in colleges and universities
- higher rates of competitive employment
- increase levels of independence
Regional Support Model
The Secondary Transition Team provides support to Administrative Units (AUs) through a regional model. Please reference the Regional Specialist Map for Transition Specialist contact information as well as training and technical assistance opportunities that are available.

Secondary Transition Taskforce
The mission of the Secondary Transition Taskforce is to improve outcomes for transition-age youth with disabilities, by bringing together a diverse range of stakeholders to collaborate, advise, and make recommendations to CDE on behalf of secondary transition service providers and young adults with disabilities and their families.
The Post-School Outcome interview process is now complete. Each administration unit (AU) attempted to contact every student in their PSO sample! As a State, our participation rate increased from 47.2% to 56%! (Participation rate is defined as "interview began" as the contact outcome.) Thank you for your hard work and all that you do for Colorado students.
